Records Request
No materials may be released from a student’s file without written authorization from the student (or student’s parents/guardians if the student is a minor or current student).
- Transcript Request: Graduates/Former Students
- Permission to Release/Obtain Information Form / Formulario de permiso para divulgar/obtener informacion
- Grades 9, 10, or 11: Records Request Form Non-College
- Grade 12: Records Release Authorization Form
Submit forms to the counseling office or to GVHSCounseling@gvsd.org.
